According to the officials, the first question is about the names of the family members. The second question is related to the name of the father or husband. The third question is to be filled one for Principal or Self, 2 for Spouse, 3 for Son/Daughter, 4 for Grand Daughter/Grand Daughter, 5 for Mother/Father. Age has to be mentioned in the fourth question. In the fifth question, information related to gender will be given. One has to be filled for male, two for female and three for others.
