Under Section 2 of the Persons with Disabilities (Equal Opportunities, Protection of Rights and Full Participation) Act, 1995, any disabled person will be considered disabled only if he is declared 40% or more disabled by the medical board. That is, if someone goes to a hospital and gets his disability checked and in the hospital, he is declared less than 40% disabled by the concerned medical board or doctor, then that person will not be able to avail the facilities being run for the disabled nor will he be able to get any exemption for work.
The medical board issues the disability certificate
If a person wants to get his disability certificate made, he can contact the hospital in his district or the hospital where he got the treatment for his disability. After this, the person has to appear before a medical board. More than one doctor or officer can be included in the medical board. After this, they examine the severity and permanence of the disability and after complete assessment, issue a certificate to the disabled person.

If a disabled person is issued a disability certificate by the medical board, then on the basis of that he can get his unique disability ID.
