You will also have to provide some documents of that member here, you can upload birth certificate or marriage card.
After this finally submit the form, after submitting the form a message will come on your mobile number. Which will contain information about your application. After a few days, a new ration card will come to your home. This ration card will also have the name of the member for whom you applied.
